Bursitis
Bursitis is inflammation of the bursae, which are fluid-filled sacs in joints. They cushion tendons, ligaments and muscles and allow them to glide over bones in the joint. In bursitis, bursae become irritated, inflamed and swollen. This hinders their function.
There are two major bursae in the hip that tend to become irritated and inflamed. One covers the bony point on the hipbone and the other is on the pelvis. It’s more common to have problems with the bursa on the hipbone.
Symptoms of hip bursitis
Signs and symptoms of hip bursitis depend on which bursa is causing problems. In general, symptoms include hip pain, tenderness, stiffness, swelling and warmth. With the bursa on the bony point, symptoms occur in the hip and extend to the thigh. The bursa on the pelvis causes symptoms in the groin.
The pain of hip bursitis tends to be worse at night, especially when lying on the hip. It can also worsen when sitting or walking for prolonged periods of time.
Causes of hip bursitis
A common cause of hip bursitis is repetitive motions or overuse injuries. This includes running, biking, or stair climbing for an extended time. Other causes of hip bursitis include hip injury, bone spurs, unequal leg lengths, spine conditions, and inflammatory conditions, such as rheumatoid arthritis. Previous hip surgery can also cause irritation of the bursa.
Treatment and prevention of hip bursitis
Treatment and prevention options focus on relieving the pain and swelling, and preventing future attacks and hip joint damage.
To start, your doctor will likely recommend nonsurgical strategies. This includes resting the hip and taking anti-inflammatory medicines. Using a cane or crutches can help relieve pressure from the hip and allow healing. Corticosteroid shots into the hip are sometimes necessary. In rare cases, your doctor may recommend surgery.
Physical therapy can help speed your recovery and prevent recurrences. A physical therapist can teach you strengthening and flexibility exercises. Other techniques include stretching your hip muscles and massaging the muscles and joint with a foam roller.
